Record Keeping Paperwork and Due Dates
All entering high school students, along with their parents, must have an individual meeting with the high school counselor in the Spring prior to the start of the school year. During this meeting, the parent(s), student, and counselor will discuss and complete the student's four year plan. Although there is an overview of record keeping requirements during the yearly high school meeting, which takes place in the Spring, the high school counselor can, if necessary, review record keeping requirements with the parent(s) and student at this time. The following are requirements for record keeping along with 2020-2021 due dates. Please take careful note of all due dates as it is important to submit all documents in a timely manner. Below you will also find links to download the necessary forms that you will need to keep and submit good accurate records.
Four Year High School Plan | Due September 8, 2020
Upon entrance into our program you will need to submit a four year high school plan and you must choose either the General Education Course or the College Prep Course. If your course plans change, then you will need to submit a new plan.
High School Course Descriptions | Due September 8, 2020
This form is due a the start of your student's school year. The courses will need to be approved by the PSP Administrator (Mary Carnighan) before any credit will be awarded.
Course Verification Form (if necessary) | January 22nd and June 4th
This form will need to be filled out and returned from any other classes that your student takes for credits where you are not the teacher.
Volunteer Verification | January 22nd and June 4th
Firm Foundation Christian Academy views volunteering as an important part of our students overall development. WE believe it is an integral aid in the process of their spiritual, emotional, and civic minded maturity. As such, students are required to complete 25 hours of volunteer work per year.
High School Quarterly Report | November 6th, January 22nd, March 26th, and June 4th
To be submitted at the close of each quarter, this report will include the subjects, pages or concepts completed, test scores and project grades, and final grade for that quarter. This form is due one week after the close of each quarter.
High School Report Card Form | January 22nd and June 4th
This form needs to be turned in four times a year--at the 1st quarter, 1st semester (combining 1st and 2nd quarters); 3rd quarter, 2nd semester (combining 3rd and 4th quarters). Transcripts will be sent to you at the end of each semester, so please be prompt with getting these in by the due date. Be sure to double check that the grades returned to you are the same ones that you recorded and sent in.
Attendance Record | Due June 4th
Parent must keep attendance, recording each day your student completes a school day. A minimum of 3 hours per day, for 175 days per school year are required.
